What sample is needed for the IgE Antibody?

  1. A venous blood sample is needed to get the serum.
  2. Random sampling can be used.
  3. Analyze a fresh sample or store it at 4 °C for less than 72 hours.
  4. Avoid lipemic or hemolyzed samples.

What are the indications for IgE Antibody?

  1. It is done to diagnose an allergy.
  2. This is a blood test that is different from the skin test.
  3. It is indicated in patients where skin test is contraindicated.

What are the contraindications for IgE Antibody?

  1. If the patient reacts to the skin test for allergy.
  2. If patients have multiple sources of allergens.

How will you describe the pathophysiology of IgE Antibody?

  1. Type 1 hypersensitivity reaction where IgE type of antibody has a role.
  2. Where allergen (Antigen) produces IgE type of antibody.
  3. IgE values are age-dependent until adulthood.
  4. Elevated values of IgE are found in allergic disorders, such as atopic dermatitis, allergic asthma, and some parasitic infestations.
  5. The raised IgE value may be seen in some non-allergic conditions, so testing the IgE level is not specific to the allergy.
  6. While allergic patients may have a normal total IgE level.

What is the radioallergosorbent test (RAST)?

  1. It was described in 1967 to detect the IgE antibody. Radioallergosorbent means to find the IgE-antibody initiated against the allergen.
  2. Principle:
  3. It is advised to measure IgE antibodies to specific antigens.
  4. In this test, a specific antigen is bound to a carrier substance and allowed to react with a specific IgE antibody.
  5. The amount of IgE antibody bound is estimated by adding radioactive anti-IgE antibodies.
Radioallergosorbent Test (RAST) principle

The Radioallergosorbent Test (RAST) principle

  1. Now, quantify the labeled anti-IgE antibody attached to the IgE-antigen complex.
  2. Using the subcutaneous injection method, RAST correlates 80% to 85% with skin testing.
  3. Normal value:
    1. 0.10 kU/L to 100 kU/L

What are the stimulants for Type 1 Hypersensitivity reactions?

Various substances (allergin) can give rise to this Type 1 reaction like:

  1. Animal dandruff.
  2. Foods.
  3. Pollens.
  4. Dust.
  5. Molds.
  6. Insect venoms.
  7. Drugs. In medication, Penicillin is at the top.
  8. Environmental pollution.
  9. Some chronic infections include fungi and parasites.
  10. Others are cosmetics and chemicals.
    1. The total level of raised IgE is the diagnostic of an allergic reaction.
    2. It can be used to diagnose allergens because of the preparation of liquid allergen preparations available.

What are the conditions where there is a significantly raised IgE level?

  1. Asthma (∼60%).
  2. Hay fever (∼30%)
  3. Dermatitis.
  4. Food allergy.
  5. Drug allergy.
  6. Occupational allergy.
  7. Latex allergy.
  8. Angioedema.
  9. Allergic rhinitis.
  10. Parasitic diseases like ascariasis, hookworm, Echinococcus, visceral larva migrans, and schistosomiasis.

When is a normal or low IgE level seen?

  1. Asthma

When will you see a decreased IgE level?

  1. Hereditary deficiencies.
  2. Non-IgE myeloma.
  3. Acquired immunodeficiency.
  4. Ataxia-telengiectasia

When will you see false Results?

  1. If patients have a disease where there is an elevated level of IgE.
  2. Corticosteroids increase the level of IgE.

You have to stop these allergins. That is the best option for cure. If you want to develop tolerance, you have to take these things in minimal doses and for a long time, and the chances are that you may develop tolerance. Net shell, my advice is to stop these things.

Your IgE level is the normal upper limit. In the case of asthma, the physician can hear wheezing. Are there any findings in the X-Ray chest? Asthma may be due to an allergy in the atmosphere. You have not given the information about the pain chest; is it intermittent or continuous?

Apologies for a long message. Well, my grandson, aged 17 years, suffered from asthma since childhood and was usually treated with Ayurvedic / Herbal medicines. At 17, he was provisionally diagnosed with ABPA and had his first corticosteroid treatment this year, which brought the IgE level down from 3000 kUA/L (Start of treatment in March 2021) to 897 kUA/L (At the end of 16-week treatment). The bio-reference intervals mentioned in the subsequent IgE reports have been somewhat confusing, as summarized below:

There is no doubt that his IgE level is raised (indicating hypersensitivity type 1 reaction). Variation from different labs is not worrying me. I will advise sticking to one lab. You should consult a pulmonologist who can check his lung functions. Ideally, finding the allergin stimulates this reaction and avoids that will lead to a cure for him. Mostly these people are treated with steroids puff.

Thanks for the prompt response and advice sir. We have been taking measures to avoid exposure to Aspergillus and it seems to be working. He has been prescribed Foracort on SOS basis, which he has hardly ever required over the past nine months.

Dear Sir, he doesn’t take other milk or restaurant food rather than BM. Al over a day; he take’s 5-8 apples(partly eat), 2-3 oranges, 20-30 gram kismis(raisin), 5-8 pieces of keshaw nut, 50-100 gram grapes, two/ three time, he take’s only rice (khichuri 1 time). He normally like to eat meat especially liver, lung without rice.
He normally sleep at 10-11 pm and wake up after 9 am.
My house is well cleaned.

Why do they advise ACE level? He needs workup for the mediastinal lymphadenopathy. They can have either FNA (Fine needle aspiration) or biopsy. However, the mediastinal area is difficult to approach. Is this mass seen on XRay’s chest?. He needs to be seen by a thoracic surgeon and pulmonologist. What is his ESR?. If his ESR is raised, he needs to take care of the mediastinal lymphadenopathy. You can ask for a tuberculin test.

Raised IgE level indicates allergy (Type 1 hypersensitivity reaction). Now you have to find the allergen. You should eat your food one by one. First exclude the oil and use butter or olive oil. keep record of your food and write all ingredients. You have allergy to some of the ingredients of your food.
